The Adult Transition Program attempts to challenge students to achieve their potential to become contributing citizens capable of meeting the demands of a changing world, by giving students training and access to a variety of resources.

The Mission of the Adult Transition Program is for all 18-21 year old students with cognitive disabilities and other significant transitional needs to have access to a continuum of supports across environments including vocational training and development, access to the community and its resources, and the opportunity to live as independently as possible.

The Adult Transition Program defines a student’s transitional plan as one that plans for realistic post-secondary outcomes. The Adult Transition Program believes that transitional planning is student centered. Aspects of appropriate transitional planning should include but are not limited to each student’s present levels of performance, educational needs, strengths and interests.
